Case Management
Case management at The Grounds involves preparing residents for success over the long term and assisting residents with developing life skills including financial planning and accountability development. Case managers also help with job skills including resume building, interviewing, workplace behavior, and career selection.
Case managers make sure their clients are attending their meetings, communicating with their sponsors and therapists, and are generally thriving within the program. They provide weekly communication with family members and status updates to ensure clients are meeting their goals and addressing any issues along the way.
Weekly Meetings
Weekly, Case Management and Life Skills Development Meetings Often Include:
- Individual coaching
- Employment coaching including resume development and interview preparation
- Financial management review and oversight
- Personal budget development
- Transitional living planning
- Transferring of bills and payments from parents to members
- Identifying and securing appropriate next-step living arrangements
- Random sobriety checks
- Random job visits
- Completing employment, college or vocational training applications
- Completing medical insurance paperwork
- Securing car insurance
- Reviewing personal credit report
- Resolving outstanding credit or billing issues
- Daily schedule reviews and time management support
- Coordination of medical, dental, therapy and psychiatric services
- Financial responsibility tasks such as taxes, identification, insurance and credit monitoring
